The U.S. is experiencing an epidemic of type 2 diabetes which will continue to worsen unless more people with prediabetes are effectively treated with preventive interventions like the Diabetes Prevention Program (DPP). Enrolling more people with prediabetes into DPPs require strong linkages between healthcare providers and the community-based organizations that offer the DPP. Technological solutions can facilitate these clinic-to- community linkages. The YMCA of the USA and the American Medical Association collaborated on a CDC-funded project to develop and test bidirectional e-referral pathways between healthcare providers and local YMCAs which deliver the DPP. In this interactive session, the speakers will share the key learning from this project.
