Connected Communities of Care Series: RoundTable
What you'll learn
A fundamental question continuing to face the healthcare industry is: How do we comprehensively address the needs of patients when those needs extend beyond the boundaries of traditional clinical care? And, the companion question is, how does appropriately addressing non-medical needs positively impact the cost, quality and value of the overall health enterprise.
These questions and others will be addressed during a 3-part upcoming web-series specifically focused on connected communities of care [CCC] and the value they bring. Lead by Healthbox’s SDOH partner, PCCI each 60 minute session will address one actionable topic where a focus on SDOH and connected communities can have an immediate impact.
